Behind the mask: LGBTQIA+ mental health, rejection & body image

LGBTQIA+ mental health is shaped by so much more than what happens in a therapy room. In this episode of Don’t Get a Therapist Yet, Wendy sits down with Rahim Thawer, a social worker, psychotherapist, author and educator, to explore the cultural forces that quietly shape how queer and trans people experience rejection, shame, body image and belonging.

Meet Rahim Thawer: Psychotherapist & Author LGBTQIA Mental Health

Rahim was born and raised in Toronto to a South Asian Muslim family who immigrated from East Africa. He has spent nearly fifteen years practising psychotherapy, teaches in a School of Social Work in Alabama, and is the author of The Mental Health Guide for Cis and Trans Queer Guys.

His work centres on the social and subcultural determinants of mental health, looking beyond individual symptoms to the communities, norms and histories that shape how queer and trans people see themselves.

Rejection, Body Image & Belonging: What This Episode Covers

Rahim and Wendy talk through the framework behind his book, exploring how LGBTQIA+ mental health is influenced by cultural norms and internalised hierarchies rather than simply whether someone is single, partnered, out or not. They discuss:

  • The idea of “subcultural determinants” of mental health, and how growing up queer in a straight world shapes identity development
  • Gestalt therapy and the empty chair exercise, and how these tools help people work through unresolved emotions and unfinished business
  • Creative adjustments and survival strategies, including how masculinity, body image and the gym culture many queer men grew up around can become both protective and limiting
  • Why Rahim prefers the phrase “inviting in” over “coming out,” and how that reframe changes the emotional weight of disclosure
  • Shame, desirability and the pressures that exist within LGBTQIA+ communities themselves, often unspoken
  • Substance use, chem sex and sexual health as their own subcultural determinant, separate from any HIV-prevention lens
  • The particular challenges facing trans people, including barriers to gender-affirming care and the emotional toll of navigating a medical system that wasn’t built with them in mind
  • Rejection, internalised hierarchies of power, and how early experiences of rejection can shape who we’re drawn to and why
  • What emotional wellness and healing can genuinely look like for queer and trans people, beyond simple symptom reduction

Who should watch this episode

This conversation goes well beyond a typical discussion of LGBTQIA+ mental health. Rahim brings a social work lens to the topic, situating rejection, shame and body image within the wider cultural and political context queer and trans people are navigating right now.

Whether you’re LGBTQIA+ yourself, a therapist, a social worker, or someone who simply wants to understand the people in your life a little better, this episode offers language and insight you won’t find in a typical self-help conversation about LGBTQIA+ mental health.
